PROGRAM COORDINATOR
POSITION OVERVIEW: The Program Coordinator is responsible for care coordination for individuals receiving supported living services. The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy. The Program Coordinator serves as an integral part of the Interdisciplinary Team (IDT), and ensures that an appropriate Individual Service Plan (ISP) is executed for the individuals on his/her caseload by completing the following: orienting and training staff on client goals and services, monitoring systems and documentation to ensure the ISP, as well as company procedures, are followed, and assisting with quality monitoring and improvement activities.
POSITION REQUIREMENTS:. One (1) year experience providing direct care to individual’s with developmental disabilities. Prior experience working with individuals with developmental disabilities, preferably in a management or administrative role. High school diploma or GED. A bachelor’s degree in social work, psychology, human services or other related field preferred. Must have strong written and verbal communication skills and able to work successfully as part of a team. Must have a current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Must be able to maintain CPR, First Aid, and medication administration certification.

PROGRAM MANAGER
POSITION OVERVIEW: The Program Manager is responsible for the day
to day management and supervision of supported living services. This
position ensures that the environment in which services are provided are
healthy and safe. The Program Manager supervises direct care staff, and
assists with ongoing training to ensure staff deliver high quality care.
This position requires a flexible schedule with the ability to take after-hours
calls.
POSITION
REQUIREMENTS: High School Diploma.
Associates or Bachelor's degree preferred. One (1) year experience managing
personnel. Excellent verbal and written communication skills are a must.
Must demonstrate proficiency in using web based applications and other
technology. Must maintain a current driver's license with less than six (6)
points and automobile insurance that meets Ohio minimum standards.

LEAD SUPPORT ASSOCIATE
POSITION
OVERVIEW: The Lead Support
Associate is responsible for delivering and supporting the delivery of high
quality services that are documented in accordance with company policy. This position provides mentoring support for Support Associates, serving as a training and information resource. The Lead Support Associate also assumes responsibility for monitoring service documentation and ensuring the assigned service location is maintained according to client and company standards,
POSITION REQUIREMENTS: One (1) year experience providing supports to individuals with developmental
disabilities. High school diploma or GED. Current
valid driver’s license with a good driving record and automobile insurance
meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and
medication administration certification. Strong communication and organizational skills a must.

SUPPORT
ASSOCIATE
POSITION
OVERVIEW: The Support Associate is responsible for providing
supervision, support and assistance to consumers as prescribed within the
Individual Service Plan/Individual Program Plan to increase independence and
quality of life.
POSITION REQUIREMENTS: Previous experience working with individuals with developmental disabilities is preferred, but not required. A High School
diploma or GED is preferred. Current valid driver’s license and automobile insurance meeting
Ohio’s minimum standards. Ability to maintain CPR and First Aid certification.

DAY SUPPORT ASSOCIATE
POSITION OVERVIEW: The Day Support Associate is
responsible for providing supervision, support and assistance to consumers participating
in our Adult Day Services Program. The
Day Support Associate ensures the activities offered at the program are
consistent with the individual’s preferences, as stated in the ISP.

diploma or GED is preferred. Current valid driver’s license and automobile insurance meeting
Ohio’s minimum standards. Ability to maintain CPR and First Aid certification.
