POSITIONS AVAILABLE:

PROGRAM COORDINATOR 

POSITION OVERVIEW: The Program Coordinator is responsible for care coordination for the individuals served.  The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy.  The Program Coordinator serves as an integral part of the Interdisciplinary Team (IDT), and ensures that the Individual Service Plan (ISP) is implemented in a consistent, high quality manner. This position is responsible for ensuring staff understand how to fulfill the ISP, and ensure services are documented as prescribed.  The Program Coordinator assists individuals with accessing outside resources, such as physicians, recreational activities, etc, in order to meet individual goals and objectives.  

POSITION REQUIREMENTS: High school diploma or GED,  bachelor’s degree in social work, psychology, human services or other related field strongly preferred.  One (1) year experience providing direct care to individual’s with developmental disabilities.   Must have strong written and verbal communication skills and able to work successfully as part of a team.  Must have a current valid driver’s license  and automobile insurance meeting Ohio’s minimum standards.  Must be able to maintain CPR, First Aid, and medication administration certification.

PROGRAM COORDINATOR (QIDP)

POSITION OVERVIEW: The Program Coordinator is responsible for care coordination for twenty-four residents residing in three ICF/DD group homes. The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective care coordination and advocacy.  The Program Coordinator serves as an integral part of the Interdisciplinary Team (IDT), and ensures that an appropriate Individual Plan (IP) is drafted and executed for the individuals on his/her caseload by completing the following: orienting and training staff on resident goals and active treatment, monitoring systems and documentation to ensure the IP, as well as company procedures, are followed, and assisting with quality monitoring and improvement activities. 

POSITION REQUIRMENTS: A bachelor’s degree in social work, psychology, human services or other related field.  One (1) year experience providing direct care to individual’s with developmental disabilities.  Prior experience working in an Intermediate Care Facility for the Developmentally Disabled and working knowledge of the rules and regulations managing ICF/DD services strongly preferred.  Must have strong written and verbal communication skills and able to work successfully as part of a team.  Must have a current valid driver’s license  and automobile insurance meeting Ohio’s minimum standards.  Must be able to maintain CPR, First Aid, and medication administration certification.

  HOME MANAGER

POSITION OVERVIEW: The Home Manager is responsible for the supervision of services for eight residents residing in an ICF/DD group home. The position ensures that the appropriate, high quality services and supports are provided to the individuals served through effective management of staff and supervision, as prescribed by the Individual Plan (IP).  The Home Manager establishes systems to ensure the effective communication of both staff and client needs, including but not limited to establishing staff schedules, communication logs, and other tracking tools.  This position routinely provides direct support to residents and works collaboratively with the team to ensure all needs are met.

ProPOSITION REQUIREMENTS: A high school diploma or equivalent.  One (1) year management experience, or five (5) years experience providing direct support to individuals with developmental disabilities, providing direct care to individual’s with developmental disabilities, with progressive responsibility.  Prior experience working in an Intermediate Care Facility for the Developmentally Disabled and working knowledge of the rules and regulations managing ICF/DD services strongly preferred.  Must have strong written and verbal communication skills and able to work successfully as part of a team.  Must have a current valid driver’s license  and automobile insurance meeting Ohio’s minimum standards.  Must be able to maintain CPR, First Aid, and medication administration certification.

SYSTEMS MANAGER

POSITION OVERVIEW: The Systems Manager is responsible for providing management oversight and staff supervision of supported living services.  The Systems Manager ensures that staff are trained and adequate in number to provide the services and supports prescribed in the individuals Individual Support Plan (ISP).  This position works collaboratively with the Interdisciplinary Team to ensure the delivery of high quality services.  This position requires a flexible schedule and the commitment to routinely provide direct support services.

POSITION REQUIREMENTS: A High School diploma or GED.  One (1) year experience in a supervisory role.  Previous experience providing supports to individuals with developmental disabilities strongly preferred.  Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.

SUPPORT ASSOCIATE

POSITION OVERVIEW: The Support Associate is responsible for providing supervision, support and assistance to consumers as prescribed within the Individual Service Plan/Individual Program Plan to increase independence and quality of life.

POSITION REQUIREMENTS: A High School diploma or GED. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.

DAY SUPPORT ASSOCIATE

POSITION OVERVIEW: The Day Support Associate is responsible for providing supervision, support and assistance to consumers participating in our Adult Day Services Program.  The Day Support Associate ensures the activities offered at the program are consistent with the individual’s preferences, as stated in the ISP.


POSITION REQUIREMENTS: A High School diploma or GED. Current valid driver’s license and automobile insurance meeting Ohio’s minimum standards. Ability to maintain CPR, First Aid, and medication administration certification. Previous experience with supporting individuals with developmental disabilities preferred.
